Possible causes
Breathlessness when horizontal can arise from cardiac problems (for example heart failure, which may cause orthopnea), lung conditions (asthma flare, COPD exacerbation, pulmonary edema) or systemic issues such as anemia, obesity-related breathing difficulty, or anxiety. Obstructive sleep apnea and reflux-related symptoms may also worsen nighttime breathing. Determining the cause requires a detailed history and physical examination.
