Vulvar pain can present as burning, stinging, sharp or constant discomfort. Causes are varied and may include skin problems, infections, trauma, or nerve-related pain. Below are common causes, key points of clinical assessment, and signs that suggest you should seek prompt medical attention.

Common causes and mechanisms

Pain of the vulva may arise from several mechanisms. Local skin irritation, contact dermatitis or eczema and fungal or bacterial infections—including some sexually transmitted infections—are frequent causes. Vaginal or bladder conditions, vaginal dryness, mechanical trauma or changes after surgery can also provoke symptoms. Neuropathic pain, such as nerve entrapment or neuralgia, is another possibility. Hormonal shifts and some systemic conditions can increase sensitivity.

Clinical assessment and possible tests

A clinician will take a detailed history about onset, duration, pain quality, associated discharge, itching, relation to sexual activity or prior trauma. Physical examination includes inspection of the external genitalia and, when indicated, internal examination of the vagina and cervix. Tests may include swabs or cultures, microscopy or pH testing, and blood tests. In selected cases a biopsy or neurological assessment may be considered. The exact evaluation depends on clinical findings.

When to seek urgent care

Seek urgent evaluation if pain is sudden and severe, if there is heavy or uncontrolled bleeding, high fever, fainting, or breathing difficulty. Rapidly worsening infection signs—spreading redness, rapidly increasing swelling—or severe systemic symptoms also warrant prompt attention or emergency services.

Daily care and reducing irritation

Simple measures can reduce irritation: cleanse the area gently with water, avoid fragranced soaps and topical irritants, choose breathable cotton underwear and avoid tight clothing. If sexual activity causes pain, pause and communicate with your partner and discuss options with your clinician. Maintaining good hygiene and following professional guidance can help symptom control.
