Vaginal bleeding

Vaginal bleeding means any bleeding that differs from a person’s usual menstrual pattern or occurs outside expected periods. Causes range from temporary hormonal changes to conditions that need prompt medical attention. This page offers general information on common causes, how clinicians approach evaluation, and when to seek urgent care or specialist review.

What counts as abnormal bleeding?

Bleeding outside the regular menstrual period can include light spotting, bleeding between periods, unusually heavy bleeding during a cycle, or any vaginal bleeding after menopause. Whether bleeding is considered abnormal depends on factors such as timing, volume, duration, and accompanying symptoms like severe pain or fever.

Common causes by life stage

Causes vary with age and reproductive stage. Adolescents often have irregular cycles and anovulatory bleeding. During the reproductive years, common causes include hormonal imbalance, contraception-related breakthrough bleeding, infections, cervical or uterine polyps, fibroids, and pregnancy-related issues (for example miscarriage or ectopic pregnancy). In perimenopause and after menopause, hormonal changes, vaginal or uterine atrophy, and less commonly neoplastic causes may be considered. A clinician interprets likely causes based on history and exam.

How clinicians evaluate vaginal bleeding

Initial assessment is usually with a family doctor or a gynecologist. Evaluation commonly includes a pregnancy test, pelvic examination, and targeted tests such as blood counts, hormone tests, swabs or PCR for infection, Pap test and pelvic or transvaginal ultrasound. The exact tests and order depend on age, symptoms, and findings on history and examination.

When to seek urgent care

Seek urgent medical attention or go to the emergency department if bleeding is very heavy (soaking a pad or tampon hourly), if you faint or feel dizzy, have very severe abdominal or pelvic pain, a fast heart rate, low blood pressure signs, or a high fever with bleeding. Any bleeding during a known pregnancy should be evaluated promptly.
