Muscle rigidity

Muscle rigidity means stiffness or increased resistance to movement in one or more muscles. It can arise from many different processes; clinical assessment is needed to determine likely causes and next steps.

How muscle rigidity is experienced

Rigidity may be felt as persistent stiffness, difficulty moving a limb, or a sense of tightness when the muscle is touched. Clinically, rigidité (continuous resistance to passive joint movement) and spasticity (resistance that varies with movement direction and speed) are distinct patterns that help clinicians narrow possible causes. Symptoms can be constant or fluctuate with activity, position, or time of day.

Common categories of causes

Muscle rigidity can result from multiple mechanisms. Frequent categories include neurological conditions affecting the central nervous system, metabolic or electrolyte imbalances, inflammatory or autoimmune disorders, primary muscle or connective tissue problems, infections, medication side effects, and overuse or strain. Associated features such as fever, pain, numbness, speech or swallowing difficulty help point toward specific causes.

Clinical evaluation and possible tests

Evaluation starts with a focused history and neurologic and musculoskeletal examination: which muscles are involved, onset and progression, triggering factors, and medication history. Based on findings, clinicians may order blood tests (electrolytes, inflammatory markers, muscle enzymes), imaging (MRI, X‑ray) and electrophysiological studies (EMG). Primary care or neurology typically coordinate these steps and decide on referrals.

Specialist guidance and when to get urgent care

Common specialists consulted include Neurology, Rheumatology, Physical Medicine & Rehabilitation, and Infectious Diseases and Clinical Microbiology when infection is suspected. Seek urgent medical care or emergency services for breath or swallowing difficulties, sudden rapid worsening of stiffness, high fever with new rigidity, altered consciousness, or seizures. These signs require immediate assessment rather than delayed outpatient evaluation.
