Impaired color vision

Difficulty distinguishing colors or a change in color perception can be present from birth or develop later. Below are common causes, how clinicians evaluate color vision, and when to seek specialist or urgent assessment.

How color vision works

Color perception depends on cone photoreceptors in the retina (sensitive to red, green and blue wavelengths) and on signal transmission via the optic nerve to visual areas of the brain. Abnormalities at the level of cones, the retinal pigment epithelium, optic nerve, or visual pathways in the brain can alter color vision. Systemic conditions, certain medications or toxic exposures and age-related changes also affect color discrimination.

Common causes

Congenital color vision differences are often inherited and commonly affect red–green discrimination. Acquired causes include retinal disorders (for example macular disease or diabetic retinopathy), optic nerve conditions (such as optic neuritis), neurological disease, metabolic or systemic illness, and drug or chemical exposures. Progressive or sudden changes in color perception should prompt clinical assessment.

Assessment and typical tests

Initial screening uses standardized plates or charts such as Ishihara plates and quick arrangements like Farnsworth D‑15. More detailed measures (for example anomaloscopy) and full ophthalmic examination may be used as needed. Imaging such as optical coherence tomography (OCT) and electrophysiological tests (ERG, VEP) can help identify retinal or optic nerve involvement. Depending on findings, referrals to neurology or internal medicine/endocrinology may be appropriate.

When to seek urgent care and everyday impact

A sudden change in color perception, especially when accompanied by vision loss, eye pain, double vision, or dizziness, requires prompt medical evaluation—seek emergency services or urgent ophthalmic assessment. Color perception differences can affect daily activities, workplace safety and the ability to identify traffic signals; specialist assessment can advise on functional adaptations and occupational implications.
