Excessive thirst

Excessive thirst (polydipsia) is a symptom that can affect daily life and arises from many different causes. It is not a diagnosis by itself; evaluation aims to find underlying metabolic, renal, endocrine, neurological or behavioral contributors.

What is excessive thirst?

Medically called polydipsia, excessive thirst means a persistent or unusually strong desire to drink fluids. It is distinct from dry mouth (xerostomia), which is a feeling of oral dryness. Clinical assessment considers onset, daily fluid intake, accompanying urine volume, nighttime waking to drink, and other symptoms such as weight change or fatigue.

Common causes and how they work

Several mechanisms can increase thirst. Elevated blood glucose (diabetes mellitus) causes osmotic diuresis and thirst; diabetes insipidus involves impaired kidney water handling; dehydration from fluid loss raises plasma osmolality; certain medications and electrolyte disturbances (for example, hypercalcemia) can also provoke thirst. Psychiatric (psychogenic) polydipsia is another recognized cause. A focused history helps narrow possibilities.

Which specialists and tests are involved?

Initial assessment is usually done in primary care or internal medicine. Basic tests often include capillary or venous glucose, electrolytes, kidney function (urea, creatinine), and urinalysis. Depending on findings, referral to Endocrinology (hormonal causes), Nephrology (kidney disorders) or Neurology may be appropriate; imaging and hormonal assays are used selectively.

When to seek urgent care

Seek urgent medical evaluation if excessive thirst begins suddenly or is accompanied by confusion, fainting, severe weakness, very high urine output, rapid weight loss, difficulty breathing, or signs that circulation or consciousness are affected. If symptoms indicate immediate danger to breathing, circulation or brain function, contact emergency services.
