Anal pain

Anal pain is a common complaint with many possible causes. Below are frequent causes, warning signs that need urgent assessment, and guidance on which specialties typically evaluate anal-region problems.

What is anal pain?

Anal pain refers to discomfort or pain felt in the anus and immediate surrounding area. It can present as sharp, burning, aching, or pressure-like discomfort. The character and duration of pain, together with any associated bleeding, discharge, swelling or fever, help guide assessment.

Common causes and how they present

Frequent causes include haemorrhoids, anal fissure (tear), perianal abscess, proctitis (inflammation of the rectal mucosa) and skin conditions such as eczema or contact dermatitis. Sexually transmitted infections, pelvic floor muscle spasm or anismus, pilonidal disease, and, rarely, tumours can also cause pain. Each condition has typical signals—for example, fissures often cause sharp pain with bowel movements, while abscesses produce local swelling and sometimes fever.

When to seek urgent care

Seek urgent evaluation for heavy or uncontrolled bleeding, high fever or shaking chills, rapidly spreading redness or swelling, inability to pass urine, or very severe pain. These situations may require emergency assessment. For other non-urgent symptoms, start with primary care or a relevant specialist.

Which specialists evaluate anal pain and what to expect

Family Medicine or primary care is usually the first contact and can arrange referrals. Depending on findings, referrals may go to General Surgery/Proctology, Dermatology and Venereal Diseases, or Infectious Diseases and Clinical Microbiology. Assessment typically includes medical history, an external examination and, if needed, anoscopy or specialist review. Questions often cover bowel habits, bleeding, discharge and sexual history.

Prevention and daily-care tips

Basic measures include good perianal hygiene, preventing constipation through a balanced diet and adequate fluids, avoiding straining during bowel movements, and limiting irritants such as harsh soaps. Practicing safer sex and protecting the skin from prolonged moisture can reduce some causes of irritation. These measures are general; persistent or worsening symptoms need professional assessment.
