Decreased fetal movements

A perceived reduction in fetal movements can be worrying. This page explains how decreased movements are commonly recognised, simple ways to monitor them at home, broad categories of possible causes, and typical clinical assessments. For personal evaluation and guidance, contact your antenatal care provider or local health service.

When to start noticing fetal movements

Many people begin to feel fetal movements around 18–20 weeks, with clearer patterns often emerging after 24–28 weeks. Individual variation is normal: some pregnancies have more frequent or stronger movements than others. What matters clinically is a clear, sustained change from the usual pattern — for example a sudden drop in frequency or strength.

How to monitor and record movements

Simple home monitoring can help identify changes. Choose a time when you are relaxed and sit or lie on your side, and note how many distinct movements occur within a set period. Many clinics use protocols such as counting a target number of movements over a defined interval. Factors like maternal position, recent food or drink, and activity can influence perception of movement.

Possible causes (general categories)

Reduced movements can reflect a range of non-specific causes: normal fetal sleep cycles, temporary maternal factors (fatigue, posture), variations in amniotic fluid volume, placental function, or fetal growth concerns. Chronic maternal health conditions may also be associated. These categories are for orientation; a healthcare assessment is needed to determine the cause.

Clinical assessment and urgent signs

Healthcare assessment typically begins with a clinical review and non-invasive monitoring. This may include a focused clinical exam, ultrasound to assess fetal growth and amniotic fluid, and electronic fetal heart rate monitoring (for example a non‑stress test) or a biophysical profile when indicated. If decreased movements occur together with vaginal bleeding, rupture of membranes, severe abdominal pain, fainting, or other worrying symptoms, seek immediate assessment at your maternity unit or emergency department.
