Skin swelling / lump

A lump or swelling felt under the skin can arise from many different causes. This page outlines common possibilities, examination findings that help distinguish them, which specialists may be involved, and signs that merit more urgent assessment. This information is educational and not a substitute for professional medical evaluation.

Common causes and brief descriptions

Subcutaneous lumps may originate from several structures. Frequent causes include sebaceous (epidermal) cysts, lipomas (fatty tissue collections), localized infections or abscesses, enlarged lymph nodes, vascular or nerve-related lesions, and, less commonly, tumors. These conditions differ in typical growth pattern, presence of pain or inflammation, and texture on palpation.

What clinicians look for on exam

Key features that help narrow the cause are the lump’s size and how quickly it developed, whether it is tender, its firmness or softness, if it moves under the skin or is fixed, whether it seems attached to the overlying skin, and any overlying redness, warmth or skin breakdown. Associated systemic signs (fever, general malaise) are also important.

Assessment steps and which specialist to consult

Initial evaluation often starts with a primary care clinician or dermatologist. Based on the exam, simple imaging—most commonly ultrasound—may be ordered to characterize the lesion. If removal or biopsy is considered, referral to general surgery, plastic surgery, or another relevant surgical specialty may follow. Decisions about further tests and management are based on clinical findings and imaging or pathology results.

When to seek urgent assessment

Urgent review is advisable when a lump grows rapidly, is very painful, or is accompanied by fever, spreading redness, increasing warmth, pus or other discharge, or skin breakdown. Lumps that interfere with breathing, swallowing, or other vital functions also require prompt evaluation in an emergency setting.
