Anal swelling is a feeling of fullness, lumpiness, or a raised area around the anus or perianal skin. It can result from several conditions; clinical examination and appropriate tests are used to identify the cause. Anal swelling may be due to haemorrhoids, perianal abscesses, pilonidal cysts, anal warts (condyloma), or benign skin cysts. Infections cause local inflammation and swelling, while persistent straining or increased venous pressure can enlarge veins and produce haemorrhoids. Each cause has distinct features—for example, an abscess is often painful and may be associated with fever, whereas warts have a rough surface.
Seek prompt medical attention if swelling is accompanied by high fever, severe uncontrolled pain, rapidly spreading redness, heavy bleeding, or sudden worsening of general condition. These signs can indicate an advancing infection or other complications; contact emergency services or attend an urgent care facility when they occur.
A clinician will take a history and perform a physical exam to assess the swelling’s location, texture, tenderness, and associated symptoms. Depending on findings, anoscopy, proctoscopy, ultrasound or laboratory tests may be used to clarify the diagnosis. Specialist assessment is often needed to determine the correct management plan.
Initial evaluation may be done by a primary care physician or a general surgeon/proctology service. Dermatology is appropriate when a skin lesion is suspected, and infectious disease input may be useful if a complex infection is suspected. The treating clinician will advise on the most suitable referral.
