Priority: If you have emergency symptoms (severe lower abdominal or one‑sided pelvic pain, fainting/lightheadedness, shoulder pain, very heavy vaginal bleeding, chest pain or shortness of breath, sudden severe headache or vision changes) go to emergency care immediately. Many people have a withdrawal bleed within a few days after finishing a pill pack; sometimes it can be delayed up to 7 days or be very light. Possible causes include hormonal effects on the uterine lining, missed or late pills, decreased absorption from recent vomiting or diarrhea, interactions with new medicines or herbal products, stress or sudden weight/lifestyle changes, and pregnancy if there was risk. These are not definitive diagnoses. If you missed doses, had recent vomiting/diarrhea, or had unprotected sex, consider a urine pregnancy test today and repeat in 48–72 hours if negative. Contact a pharmacist or your prescriber for advice on how to continue the pill. If bleeding doesn’t start by 7 days, a pregnancy test is positive, or you have concerning symptoms, see an Obstetrics and Gynaecology specialist.
