• A clear photo or typed list of all test values (each test with its units)
• The laboratory reference ranges (if shown alongside values)
• Test date and whether you were fasting
• Why the test was ordered (symptoms, follow-up, routine check, etc.)
• Any regular medications, supplements or over-the-counter products you use
