Seek urgent medical evaluation without delay for sudden severe pain, high fever, rapidly spreading redness or swelling, or new difficulty walking or urinating—these can indicate a more serious infection.

Causes and risk factors

Most perianal abscesses start when an anal gland becomes blocked and infected. Risk factors include inflammatory bowel disease such as Crohn’s, trauma, recent surgery, immune suppression, diabetes, and obesity. Recurrent abscesses make investigating an underlying cause particularly important.

Diagnosis usually begins with a clinical examination. Additional tests—anoscopy, ultrasound, or MRI—may be used to map an abscess or fistula. Initial contact is often with primary care or an emergency department; definitive assessment and procedures are typically performed by General Surgery or Colorectal Surgery (proctology). For recurrent disease or suspected underlying bowel disease, Gastroenterology or Infectious Diseases may be involved.

Complications, recurrence, and self-care tips

If untreated or when underlying disease exists, abscesses can recur and a chronic fistula may form. Rarely, infections can spread to surrounding tissues. General measures that may reduce risk include maintaining perianal hygiene, preventing constipation through diet and fluids, and avoiding prolonged straining. Persistent or recurrent symptoms should be evaluated by a clinician.
