What is hyperthyroidism and common causes?

Hyperthyroidism occurs when the thyroid gland produces more thyroid hormones than the body needs. Common causes include Graves disease, toxic multinodular (toxic nodular) goiter and subacute thyroiditis. Elevated hormone levels can affect heart rate, energy use and many organ systems.

Common signs and symptoms

Typical symptoms include a rapid or irregular heartbeat, unintentional weight loss, increased sweating, hand tremor, anxiety or nervousness, sleep problems and hair thinning. Some people experience diarrhea, menstrual changes or muscle weakness. Graves disease may cause eye changes (protruding or dry, red eyes), which often require assessment by an eye specialist.

Diagnosis — tests you may be offered

Initial testing usually includes blood tests for TSH and free T4 (and sometimes free T3). Antibody tests can help identify autoimmune causes. Imaging such as thyroid ultrasound or nuclear medicine scans may be used when indicated. Primary care or internal medicine commonly start the evaluation; diagnosis confirmation and ongoing management are typically handled by an endocrinologist.

If you experience severe chest pain, marked shortness of breath, fainting, very rapid or irregular heartbeat, high fever or confusion, seek emergency medical care promptly. Endocrinology manages most follow-up; ophthalmology is involved when eye signs are present and surgical teams may be consulted if surgery is being considered.
